Unsafe Method of Rope Work Resulted in Severe Hand Injury 不正确的操作揽绳会导致手部严重受伤

One of the ABs was spooling a mooring line from the starboard locker to the starboard tugger winch aft. The AB caught his fingers

 between the crow bar and roller which lead to injuries to three of his fingers including loss of one joint of finger. Source: MSF

 

Pallet Strop Hook Broke and Hit AB 挂索的断裂导致水手受伤

The pallet strop hook was incorrectly secured to the crane hook making it much weaker than the limits of the lifting strops 

SWL. The AB was severely injured. Source: MSF

 

Hit by Mud/Clay During Anchor Handling Operations  抛锚操作中被/土击伤

During recovery of anchors from the seabed a marine rep was injured after he got a 50 kg lump of mud/clay rolled onto his back

 as he was checking the Identification number of a shackle. Source: MSF

 

Crewman Fell on External Stairway 海员坠落悬梯

Crewman had just completed recovery of the vessels daughter craft (DC) and had informed the Officers in the bridge 

that the DC had been recovered and stowed safely on-board. His left arm came into contact with the stairs. Source: MSF

 

Unsafe Boarding of Vessels 不安全的登船方式

The surveyor removed the chain and jumped from the quayside onto the ship. The aft mooring station had only just confirmed 

the vessel was all fast to the bridge and the engines and thrusters were still running when the incident occurred. Source: MSF

 

Injury to Crew Member 海员受伤

The qualified 3/E accepted the task and proceeded to the engine room. The engines on board had a lower temperature and a 

higher temperature header tank. Both required topping up. During work; injury happened. Source: MSF

 

Serious Finger Injury During Valve Installation安装阀门时导致的手指受伤

The incident occurred during valve operation function tests on a remotely operated pneumatic controlled valve for a vessel

Failure of Limits, Stops and Safeties 限位器、制动装置失灵

The incident happened during monthly testing of fire-fighting equipment.
